Attach complete copy of PHA Plan to be Certified - Required - Invalid File Type

Attach completed Certification by PHA Official form. The form is available HERE - Required - Invalid File Type

Attach 50077-SL form completed for KHC signature. - Required - Invalid File Type

If requesting certification of an amendment to an existing PHA plan, please attach a bulleted summary of all amendments. - Invalid File Type
